FIN:        I0876-3   - Patch 112276-06 (Firmware 2.01.03) and later for 
                        Sun StorEdge T3+ (T3B) Arrays resolves several 
                        disk error handling issues.

Issue Description: 
Sun StorEdge T3+ Arrays with firmware version 2.01.01 and Sun StorEdge 
3900/6900 Series systems with Service Processor (SP) Image 2.1.2 are 
susceptible to loss of volume access or data loss due to a disk error 
handling issue.

Affected units may be identified as follows:

Note that T3 (T3A) arrays are not affected by this issue.

   T3+ (T3B)
   ---------
   Affected arrays will have firmware version 2.01.01 installed.  
   To verify the current firmware version running on a target T3+ system, 
   use the 'ver' command at the T3+ command line as follows:
 
      t3:/:<3>ver

   3900/6900 Series
   ----------------
   Login to the SP and type: "cat /etc/motd".  The response on the SP 
   should indicate that the revision of the Service Processor image 
   is 2.1.2.  Only SP image revision 2.1.2 contains T3+ firmware 2.01.01 
   and must be upgraded.
  
Depending upon the

   1) configuration,
   2) application,
   3) type of volume manager,

in use, the host may

   1) continue the retry read/write operations,
   2) unmount the volume,
   3) cause the application to timeout.

If a disk drive experiences one of the following errors,

   "Sense Key = 0x4",
   "Sense Key = 0x01, Asc = 0x5d"

the T3+ will repeatedly retry read/write operations.  This may appear to 
the host as if the T3+ is not responding.  

If more than one disk in a given RAID volume reports these particular 
errors, then ALL of the drives that report these errors WILL be disabled.
This will result in the volume being unmounted and will cause a loss of 
access to data.
  
T3+ FW 2.01.03 and above has been enhanced to properly disable and 
handle occurrences of the 0x4 and 0x01/0x5d SCSI errors.

This issue is addressed with T3+ firmware patch 112276-06 (or later).
This firmware is available for the 3900/6900 Series in SP Image 2.3.1
(or later).  To avoid the potential loss of access to data, special 
pre-installation and post-installation procedures are required with 
patch 112276-06 and are detailed in FIN I0876-3.

Corrective Action: 
The following recommendation is provided as a guideline for authorized
Sun Services Field Representatives who may encounter the above
mentioned issue.
  
For T3+ (T3B)
-------------

   Install firmware patch 112276-06 or later.  FIN I0876-3 provides details 
   for this procedure and must be completely followed for the Corrective 
   Action.
    
For 3900/6900 Series
--------------------

   Identify whether your customer is running SP image rev 2.1.2.  Login to 
   the SP (Service Processor) and type: "cat /etc/motd".  The response on 
   the SP should indicate that the revision of the Service Processor image 
   is 2.1.2.  Only SP image 2.1.2 contains T3+ firmware 2.01.01 and must be 
   upgraded. 
     
   Upgrade the Service Processor Image to rev 2.3.1 or later by accessing
   the image and following the README file for upgrade information at:

Once the Service Processor upgrade is completed, the T3+ controller 
firmware must be upgraded as a separate process.  It is very important to 
follow FIN I0876-3 before installing the T3+ firmware upgrade patch on  
3900/6900 Series systems.  The T3+ upgrade process is explained in the
patch document and in the image README file.  It is also explained in the
Sun StorEdge(tm) 3900 and 6900 Series Reference and Service Manuals,
816-5253-11.

i)   In case of MANDATORY FINs, Sun Services will attempt to contact   
     all affected customers to recommend implementation of the FIN. 
   
ii)  For CONTROLLED PROACTIVE FINs, Sun Services mission critical    
     support teams will recommend implementation of the FIN  (to their  
     respective accounts), at the convenience of the customer. 

iii) For REACTIVE FINs, Sun Services will implement the FIN as the   
     need arises.
