Issue Description: 
The OBP may not detect the SunPCi II or SunPCi IIpro card in the PCI
slot following rapid, successive power cycling of the system making
these cards inaccessible.  Any applications that are dependent on these
cards and/or their associated devices will become unavailable.

During rapid power cycling, the Voltage Regulator on the card can
overheat, resulting in a card failure.

A limited number of SunPCi II and SunPCi IIpro cards shipped between
April 23rd 2001 and May 18th, 2001 are affected.

Root cause analysis at Sun's manufacturing vendor isolated the source
of the failures to a bad batch of voltage regulators.  There is no
correlation between a board's serial number or date code.  The only way
to determine if a board is affected is as follows.  The affected
voltage regulators have a datecode of "0038".  This regulator can be
found at board location U6.  In order to check the datecode of the
voltage regulator the heat sink that covers this component must be
loosened.  If this Semtech voltage regulator has a datecode of "0038"
it could experience this failure mode.

Corrective action was put in place within Sun Manufacturing on August
1, 2001 via Worldwide StopShip/Purge P001-20001, and Enterprise
Services on November 7, 2001 via DPCO 265.

Upon Failure replace 375-0131-04 with 375-0131-04 (with stopship label
P001-20001).

Upon Failure replace 375-3051-01 with 375-3051-01 (with stopship label
P001-20001).

Reworked spares will have DPCO Label on FRU Carton.

 Identification of Suspect Part
 ------------------------------
Affected SunPCi II cards (Sun p/n 375-0131-04) will have a serial
number between 011957 and 016784, and will have a Semtech voltage
regulator with date code 0038.  This regulator can be found at board
location U6.

Affected SunPCi IIpro cards (Sun p/n 375-3051-01) will have a serial
number between 000267 and 001043, and will have a Semtech voltage
regulator with date code 0038.  This regulator can be found at board
location U6.

               Sun is available based on the On-Site Labor Rates
               defined in the Price List.

i)   In case of Mandatory FCOs, Enterprise Services will attempt to contact
     all known customers to recommend the part upgrade.

ii)  For controlled proactive swap FCOs, Enterprise Services mission critical
     support teams will initiate proactive swap efforts for their respective
     accounts, as required.

iii) For Replace upon Failure FCOs, Enterprise Services partners will implement
     the necessary corrective actions as and when they are required.
