Solution

Resolution
One of the two blade contacts was not inserted into the mating socket properly. This is not readily apparent but can be found upon close inspection. This affects only the second generation of rail terminators denoted by the use of the HBQ card.
It is also possible to have intermittent rail communication errors if the terminators are not assembled correctly.
The part number for the HBQ terminator card is 313823202.
