Symptoms
Format will not recognize any devices off the Jasper card after patching.
The boot shows the card in /var/adm/messages but no devices get built.

This affects the Sun StorEdge[TM] PCI Dual Ultra3 SCSI Host Adapter.

Resolution
Three options:
First:
If this problem is occuring with a Sun v440 and Solaris[TM] 9,
Run: "showrev -p | grep 115665"
If you do not have the -06 or later patch installed, please download and
install it, then reboot.  Patch version -03 to -05 are known to
break qus functionality.
Second:
Please check the /etc/driver_classes file for the following entry:
qus     scsi
If the entry is not there , add it. (separated by a tab)
Reboot afterwards. If entry is there, then goto the Second option.
Third:You need to remove and readd the following packages...
SUNWqus
SUNWqusu
SUNWqusx
SUNWqusux
Solaris[TM] 8 Patch
112697-xx
Solaris[TM] 9 Patch
