Symptoms
Configuration: E6500, Solaris[TM] 2.6 with KJP 33, VxVM 2.6, Sun Cluster 2.2, A5200 x 2 as shared storage.
Problem:
A disk drive in A5200 failed, "luxadm insert_device" hang during disk replacement. The disk replacement procedure can't be finished.
root@smp1 # luxadm insert_device A,r4
The list of devices which will be inserted is:
1: Box Name:    "A" rear slot 4
Please verify the above list of devices and
then enter 'c' or <CR> to Continue or 'q' to Quit. [Default: c]:
Hit <Return> after inserting the device(s).
Drive in Box Name "A" rear slot 4

The command "luxadm insert_device" does not return, it just hangs there.
Command "format" can't find the newly replaced HD, and "luxadm display" shows that no path found for the HD(WWN:2000000c50ac8166).

Command "ps" shows that "luxadm insert_device" hangs with child process "drvconfig -i ssd".
Power cycle the A5200, reboot E6500 server with "-r" option, server failed to boot up.


Resolution
The symptoms match that described in:
Sun Alert 228522
According to the Sun Alert, the resolution is to upgrade KJP to 105181-34 or later.

Relief/Workaround
In our case, we followed the workaround according to Sun Alert 48586, booted the server without "-r" option, added the following entry to the /etc/system file:

set kobj_map_space_len=0x200000

then rebooted the server with "-r" option, the problem solved, we then finished the disk drive replacement procedure.
